Endophthalmitis after cataract surgery in Sweden 2018-2024: associations with preoperative visual acuity and intracameral antibiotic regimens

WHAT IS ALREADY KNOWN ON THIS TOPIC * Postoperative endophthalmitis (PE) after cataract surgery is rare, and intracameral (IC) cefuroxime or moxifloxacin is widely used for prophylaxis, but comparative evidence between regimens remains limited. The association between preoperative visual acuity (VA) and the risk of PE has not been well established.WHAT THIS STUDY ADDS * In nearly one million Swedish cataract procedures, PE occurred in 0.013%; male sex, ocular comorbidity and capsule complication were associated with higher odds, whereas preoperative VA≥20/40 and immediate sequential bilateral cataract surgery were associated with lower odds. No IC antibiotic regimen was independently associated with a lower incidence, although microbial findings and clinical outcomes differed between regimens.HOW THIS STUDY MIGHT AFFECT RESEARCH, PRACTICE OR POLICY * Preoperative VA and case mix should be considered when comparing endophthalmitis rates between populations, while larger studies with improved control of confounding are needed to determine the optimal IC antibiotic regimen.

New research and our colleague Christiaan Mulder mentions that these findings highlight a critical opportunity: #TB programs need to look beyond treatment initiation and strengthen person-centred support throughout the entire TPT journey. 🔗Read the article here: academic.oup.com/ofid/art….
